of New York. Only families new to
Incarnation School qualify for the

apply to Incarnation School,
please fill out the information
request form below and someone
from the school will contact you
in 2 to 3 days (if you are not
contacted please call the school
at (212) 795-1030). Your child
must either be screened (Pre-K
through 1st) or tested (2nd
through 8th), or meet with the
principal before being accepted.
All potential students
will need the following:
» Birth certificate and proof of
residency (Con Ed bill, rent
stub, etc)
» Social Security card
» Immunization records
» Report card and standardized
test results from previous
school
» Baptismal records (if
applicable)
